Malaysia is two trips in one. Over on the peninsula you've got the buzz of Kuala Lumpur, the food and heritage of Penang and Melaka, the tea plantations of the Cameron Highlands and the islands off the east coast. Then there's Malaysian Borneo — Sabah and Sarawak — with Mount Kinabalu, the orang-utans and the deep jungle. The two halves can call for slightly different travel-health advice, which is why a quick chat before you go is so handy.

Peninsula or Borneo? Why it changes the advice

For a fairly standard trip — Kuala Lumpur, Penang, Melaka, Langkawi and the east-coast islands — most travellers are looking at hepatitis A, typhoid and a tetanus, diphtheria and polio booster if theirs has lapsed. That covers the great majority of holidaymakers on the peninsula. Borneo and the deeper rainforest is where the extras start to matter more. Longer stays, jungle lodges, river trips and time well away from a hospital push rabies, hepatitis B and Japanese encephalitis up the list, and bring malaria into the conversation. Bring a rough itinerary and we'll tailor it to you rather than jab for the sake of it.

Malaria & mosquito bites

Malaria risk in Malaysia is low and very patchy rather than countrywide. There's no meaningful risk in the cities, the main tourist areas of the peninsula or Penang, so most city-and-beach holidaymakers won't need antimalarial tablets. Where it does come into play is the forested interior of Malaysian Borneo — parts of Sabah and Sarawak — where a monkey-carried type of malaria (Plasmodium knowlesi) is found, so tablets may be advised for jungle and rural trips there. Because it's so route-dependent, tell the pharmacist exactly where you're going and we'll give you a clear answer. Whatever we decide on tablets, good bite avoidance protects everyone: a DEET-based repellent, loose long sleeves and trousers in the evenings, and a net or air conditioning at night.

Dengue and Zika — the daytime biters

Dengue is the one to take seriously in Malaysia. It's present all year round, in towns and cities as well as rural areas, and is spread by mosquitoes that bite during the day. There's no routine vaccine we'd give for it, so daytime bite avoidance is your main defence — the same repellent-and-cover-up routine, not just at dusk. Zika and chikungunya are also present. Zika is usually mild but can affect a pregnancy, so anyone who is pregnant or trying to conceive should have a word with us before booking, as current advice may be to reconsider travel. The good news is the same precautions work against all three, so getting into the habit early pays off.

Do I need a yellow fever certificate for Malaysia?

Yellow fever isn't found in Malaysia, so there's no health risk from it there and you don't need the vaccine to protect yourself. A certificate only comes into it if you're arriving from — or have recently passed through — a country where yellow fever is a risk, such as parts of Africa or South America. Flying straight from Manchester or anywhere in the UK, you won't need one. When to book your appointment

The earlier the better. A few courses, such as rabies or hepatitis B, are given as more than one dose over a few weeks, so ideally pop in around four to six weeks before you fly. If Malaysia has crept up on you, don't write it off — a last-minute appointment is still well worth it, as there's usually plenty we can do and we'll prioritise the jabs that matter most for your trip.

Almost certainly not. The cities and main beach resorts on the peninsula are not a malaria area, so tablets generally aren't recommended for that kind of trip. We'd still suggest good daytime bite avoidance because of dengue, and the pharmacist will confirm based on your exact plans.
